Innovative Approaches to Treating Compound Fractures of the Leg: A Plastic and Reconstructive Surgeon's Perspective

ir="ltr">In this article, Dr. Leena Jain shares her insights on treating compound fractures within her specialty's purview. The complexities of compound fractures, including an open wound with bone piercing the skin, prove challenging. However, a comprehensive and multidisciplinary approach, which we will delve into, becomes essential, providing a thorough and reassuring treatment plan for the patient.

Dr. Leena Jain, a renowned plastic and reconstructive surgeon from Mumbai, India, states, 'Treating compound fractures requires considerable surgical skills and the vision to foresee and deliver an outcome that will normalize the patient's life in the best possible manner.  Right from the initial treatment to the surgical repair and restoration followed by extensive rehabilitation, the role of a reconstructive plastic surgeon is extremely critical.'

In addition to her private clinic in Borivali, Dr. Leena Jain is available at the Lilavati Hospital in Bandra, Mumbai. Having acquired two esteemed fellowships from Germany and Korea, she has evolved into an excellent plastic surgeon with expertise in hand reconstruction.

Ms. Vishaka Goyal, Co-founder of ClinicSpots, a doctor discovery platform, states, 'I completely trust Dr. Leena's surgical acumen and capabilities. From planning the surgery to fully recovering the patient in terms of functionality, Dr. Leena Jain covers the patient's case holistically, leaving no aspect untouched. She is very committed to patient care and recovery.'

Across over eight years of rich experience in this field, she states, 'Like all other types of injuries, compound fractures require intensive and meticulous treatment, considering the need to have a multi-faceted approach to treat the patient in their injured condition.'

After first aid, the surgeon checks the injury to clean the wounded site, including removing the contaminated and non-viable tissue to reduce the infection risk and encourage healing. This process is known as debridement.

After debridement, the surgical team performs the internal fixation, a procedure that requires a great deal of planning. Internal fixation involves the use of plates, screws, and metal rods to hold the bone in place and give it stability. The choice of these fixation devices and their placement is crucial for the successful healing and stability of the bone.

As a reconstruction surgeon, my role involves reconstructing the soft tissues of the wounded area. Depending upon the extent of the surgery, the patient could require skin grafts, free flaps, or local flaps to cover the wound area for healing and aesthetic purposes. The surgeon considers the extent of injury, available tissue, choice of technique, and other key factors when planning the reconstruction procedure.

The role of a reconstruction surgeon extends to wound care and monitoring for signs of infection or complications such as nerve damage, nonunion or malunion, tissue necrosis, etc. The routine can involve scheduled dressing changes and wound assessment. Pain management is vital for recovery and patient comfort. Also, rehabilitation toward functioning and mobility requires in-depth planning, considering the patient's condition and needs.

She shares a patient story, Rita, a 23-year-old, young avid biker who unfortunately met with a mountain biking accident with a compound fracture in her left leg. Preliminary investigation revealed a great deal of bone and tissue damage. The road to recovery was challenging and long.  With immense faith in Dr Leena Jain, the patient embarked upon her treatment journey, which involved multiple surgeries and wound care. The phases of her treatment included complex procedures involving bone, muscle, and skin repair, as well as tissue reconstruction. With extensive rehabilitation, Rita restored the strength and mobility of her left leg. After nearly six months, Rita could cycle and even go mountain biking.

Today, Rita is leading an active lifestyle thanks to Dr. Leena Jain's expertise in reconstructive surgery and her perseverance in getting Rita back on her feet and even biking to the fullest possible capacity.

To conclude, Dr. Leena Jain emphasizes the need to carefully choose a reconstructive surgeon, especially if there is a need for flap reconstruction when treating a compound fracture. An experienced surgeon focuses on restoring the function and form of the injured area keeping in mind to prevent long-term complications and improve the patient's quality of life.
